NEW IMMUNIZATION REQUIREMENTS FOR THE 2010-2011 SCHOOL YEAR

Please note that the Ohio Department of Health has changed the immunization requirements for students who will be entering Kindergarten and for students who will be entering 7^th grade in the fall of 2010:

• All Kindergarten students will be required to have two doses of varicella (chickenpox) vaccine
• All 7^th grade students will be required to have the Tdap which is a tetanus, diphtheria and pertussis booster
  • Any student who received Td or Tdap for an injury or other reason since September 2005 will meet this requirement and not need to be re-immunized for entry to 7^th grade.

These immunizations must be received prior to the start of the 2010-2011 school year. As with other school immunization requirements, Section 3313.671 of the Ohio Revised Code states that students who do not provide documentation of this immunization are subject to exclusion from school.

Please contact your health care provider for this immunization. Alternatively, the Cuyahoga County Board of Health offers low cost immunizations for children at its clinic located at 5550 Ventura Dr. in Parma. They can be reached at 216-201-2041 for an appointment.

To avoid exclusion from school, it is important your student receive this immunization before school starts in the fall. Documentation must be mailed to the address below or given to the school secretary by September 1, 2010.
